Service Description: The team provides access to primary care for people who may be unhoused/unsheltered and have other complex health concerns such as mental health and substance use and are unattached to primary care or with barriers in accessing appropriate health care support * no health card (OHIP) is required to access the service * team include nurse practitioners and a doctor, registered nurse, social worker, and peer support worker * also offers education and training sessions, group-based activities as well as specialty health clinics to community members

Services include (but are not limited to):
  • health check-ups
  • blood work and tests
  • vaccinations
  • sexual health testing
  • wound care
  • smoking cessation
  • naloxone and safer drug use equipment
  • addictions treatment
  • counselling and peer support
  • service and specialist referrals
  • medical forms
