Offers a full range of supportive housing options for those with mental health and/or substance use issues – from highly intensive staff supported residences to community supports through rent subsidies with private landlords * staff team supports multiple needs and works to strengthen essential life skills * staff also assist in connecting with additional supports in the community such as interdisciplinary health teams * supports individuals to transition to more independent housing Staff supported residences include: Grace House

Admission by referral only * referrals are accepted from community agencies, professionals, psychiatrists, and local hospitals, educational facilities, doctors, through one-Link referral source (form downloadable from website) * telephone or personal contact must be followed by the referral in writing from appropriate social agency * specific application and referral forms will be forwarded once one-Link referral received |
